East Health Trust PHO Ltd provides primary healthcare services in the east Auckland area and Pukekohe to an enrolled population of approximately 120,000. We are excited and proud to be launching an innovative new primary care workforce model, the Comprehensive Primary Care Team (CPCT). These roles are aimed at driving more equitable health outcomes by increasing overall access to services, through integrating a range of new role types into existing general practice teams.
About the model:
This new, model integrating CPCT into existing general practice teams is intended to help reach more of our at-risk communities, reducing barriers to accessing care by bringing different allied health, social work, and physiotherapist-based professionals directly into the practices. This pilot will allow for free access for patients to a range of crucial services, alleviating some of the strain on the wider GP team. This is intended to enable better integration of on-site whanau-facing care, while also offering much needed additional capacity and workforce to support our heavily stretched primary care teams.

About the role
We are looking for a full-time (40 hours per week) pharmacist to work at Pukekohe Family Health Centre with the responsibility to provide care across the comprehensive primary and community team for people and whānau, who are at greatest risk of poor health outcomes and to actively support hospital avoidance.
The practice pharmacist will work as an integral part of the Comprehensive Primary and Community Team (CPCT) and Interdisciplinary team (IDT) and as a key member of the IDT, will be actively involved in IDT structure and disciplines including meetings and using the systems and process that support IDT functioning and outcomes.
The practice pharmacist will provide care directly to people and their whanau, in addition to population-level and practice-facing activities that support the comprehensive primary and community interdisciplinary care team to provide accessible, equitable and optimal use and management of medication treatments.
Responsibilities :
Medicine management and optimisation to improve medicines-related health outcomes.
Clinical evaluation and management of medication use with regards to potential or experienced risk of medication-related harms and medication efficacy to determine continued appropriateness, for ongoing management options.
Medication reconciliation to ensure safe and effective medicine use.
Supporting management of long-term conditions through initiation and continuation of prescribed treatments within area of practice (Prescriber scope only).
Improve awareness and management of medication adherence to health and wellbeing and medicines related harms.
Practice/ system directed activities – medication utilisation evaluation, audit and quality improvement for the delivery of best practice and care.
Skills and experience
Bachelor of Pharmacy (BPharm) or equivalent overseas qualification
Qualified pharmacist with current annual practising certificate (APC) without restrictions
Pharmacists undertaking prescribing activities must be registered and have a current APC without restrictions in the Pharmacist Prescriber Scope of Practice.
Holds or working towards, relevant postgraduate clinical pharmacy qualification (preference to Postgraduate Diploma level or equivalent).
Demonstrate cultural competence and understanding of Te Tiriti o Waitangi in action.
Comprehensive understanding of the inequities in access and use of medicines in Aotearoa, New Zealand and its causes
Advanced written and verbal communication skills
Excellent time management and organisational skills
Excellent critical appraisal skills, being able to identify the best evidence-informed solutions to clinical and practice questions and issues.
Minimum of two years recent experience in working within interdisciplinary clinical care teams – in primary and hospital settings.
Experience in leading and advising other health professionals or providers of care.

Have excellent organisation and time management skills alongside an ability to work independently and to collaborate with the interdisciplinary team when required.
Understand the principles of Te Tiriti o Waitangi and respectful of whanau personal beliefs as well as those of other cultures.
Be able to engage and communicate in an appropriate and respectful manner with Maori, Pakeha, Pasifika and other cultures.
Establish and maintain positive working relationships with people at all levels within the public and private sectors, related industry and community interest groups.
Develop and maintain relationship within the practice team, wider community, secondary care providers, external agencies and NGOs.
Can formulate a clinical judgement and course of action following a clinical encounter, that incorporates the needs and aspirations of individuals and whanau.
Demonstrate the highest standards of personal, professional, and institutional behaviour through commitment, loyalty and integrity.
