The Clinical Nurse Specialist – Older Adult position presents an exciting opportunity for a highly motivated senior nurse dedicated to the care of older individuals. As a CNS Older Adult, you will utilise your expertise in gerontology nursing assessment and diagnostic reasoning to assess frail older patients in the hospital inpatient setting, particularly those at risk of requiring extended care due to pre-existing functional challenges.

Working within a supportive team environment, you will conduct Comprehensive Geriatric Assessments and collaborate with medical, Allied Health, and nursing teams to coordinate clinical care, rehabilitation, and discharge planning in the acute care setting. Additionally, you will contribute to continuity of care between the PARIS team in ED/APU, the HIA ward, and the Community Geriatric Service, while also participating in educational initiatives.

To succeed in this role, our preferred candidate will display the following:

Have significant experience with Older Adults.

Registered Nurse with a current Annual Practicing Certificate.

Postgraduate academic papers in Gerontology including Assessment and Diagnostic Reasoning would be an advantage.
